Office Hours and Appointments
Our office hours are from 8:30 a.m. to 5:00 p.m., Monday through Friday. Please call 920-468-0246 to schedule an appointment or any other questions you may have.
Dial 911 if you have a medical emergency. If you have other questions for us after our normal office hours, dial 920-468-0246. Our answering service will page the physician on call.
Information For Your Appointment
Please bring your x-rays or other exams, as well as the reports of these exams to your appointment. The exams may be in the form of films or CD’s. We can interpret either. Please bring along your insurance card, and referral slip if required by your managed care company.
Visit the Patient Forms section of the website and print the Registration Form and Medication List Form, fill them out ahead of time and bring them to your appointment.
